Job Description

The role of the Nursing Associate/Assistant Practitioner is to work with the Registered Nurse and other members of the multi-professional team, providing and monitoring care and contributing to the ongoing assessment, as agreed with supervising clinician(s) and in line with the service pathways.

They will monitor the condition and health needs of people within their care on a continual basis in partnership with the multi-professional team. The Nursing Associate/Assistant Practitioner has a breadth of knowledge across the lifespan, providing holistic and person-centered care and support for people of all ages and in a variety of settings.

They will help to bridge the gap between Health Care Support Workers/Nursing Assistants and Registered Nurses by working independently under the leadership of Registered Nurses, working within the sphere of nursing and care and within all aspects of the nursing process.

Nursing Associates are equipped with the knowledge, understanding, skills, attitudes, and behaviors relevant to employment; and will act in accordance with the NMC Code: Professional standards of practice and behavior for nurses, midwives, and nursing associates.

The nursing associate/Assistant Practitioner will provide nursing care to children with complex medical needs in Special Schools or within other community settings. This is a 5-day-a-week service.


  • Provide compassionate, safe, and effective care and support to children in a range of care settings, including home, hospital, and school.
  • Monitor the condition and health needs of patients within their care on a continual basis in partnership with patients, families, and carers, ensuring appropriate escalation to a Registered Nurse when required.
  • Support children/young people and their families to improve and maintain their mental, physical, and behavioral health and well-being.
  • Care for a patient’s nursing needs whilst promoting comfort and well-being, ensuring their personal and social needs are met.
  • Safely undertake procedures to meet the patient’s nursing needs for which you have attained competence, for example, recording and interpreting vital signs, continence care, bladder, and bowel care, pressure area care, wound care, nutrition, hydration, foot care, and skin integrity.
  • Work with the Registered Nurse and engage with other health and social agencies to ensure a safe, individualized approach to the care of the child/young person.
  • Maintain accurate, contemporaneous patient records in line with Trust policy and the NMC Code.
  • Provide effective evidence-based care for service users under the supervision of Registered Nurses in order to perform nursing activities, in line with clinical governance and professional standards.

Education / Qualifications

Essential criteria:

  • Registered Nursing Associate on the NMC Register with a Foundation Degree or Assistant Practitioner with Foundation Degree.
  • GCSEs Grade C or above in Maths and English Language / New GCSE Grade 4-9 in Maths and English Language OR Functional Skills Level 2 in Maths and English or Equivalent (not entry level 2).


Essential criteria:

  • Understanding of the scope of the role of the Nursing Associate/Assistant Practitioner in the context of the nursing and interdisciplinary team and the organization, and how the role may contribute to service development.

Desirable criteria:

  • Knowledge of complex conditions and how this can impact the whole family.

Training and Experience

Essential criteria:

  • Experience working in teams under appropriate supervision as part of a multi-disciplinary team.
  • Insight into how to evaluate own strengths and development needs, seeking advice where appropriate.
  • Experience in providing and receiving complex, sensitive information,.Desirable criteria.
  • Experience in caring for people with complex health conditions.
  • Community Experience.
  • Experience in caring for children.

Skills and Abilities:

Essential criteria:

  • Ability to organize and prioritize own delegated workload; recognize where elements of care can safely be delegated to other colleagues, carers, and family members.
  • Ability to communicate effectively (written, digital, verbal, and non-verbal communication) with patients/relatives and carers and all members of the multi-disciplinary team.
  • Ability to develop effective and appropriate relationships with people, their families, carers, and colleagues.
