Connected Care Clinic at UHN

The Connected Care Clinic focuses on delivering a virtual integrated care experience for patients and their caregivers. Our goal is to improve patient satisfaction and overall health outcomes by following the patient throughout their journey, including supporting individuals in staying at home or as close to home as possible.

During the pandemic, the Connected Care Clinic was established to deliver comprehensive COVID care. This clinic allows patients to be monitored at home while drawing on the expertise and capacity of home & community care supports and primary care.

In addition to continued COVID Care for patients (including Long COVID and therapeutic treatments), the clinic is now being expanded to support clinical surges (e.g. influenza, pneumonia, and chronic conditions support) and be an integration hub for the community to support primary, home, and community care providers.
